Please take a moment to read and complete any and all appropriate forms for your pet's upcoming visit. Anesthesia/Surgical Consent Form

If your pet has an upcoming anesthetic or surgical procedure with us, please take a moment to read our Anesthesia/Surgical Consent Form in its entirety and complete the highlighted areas. Please do not hesitate to contact our office if you have questions about completing this form. Don't forget to bring this release form with you on the day of your pet's procedure.

Boarding Agreement

If your pet has an upcoming boarding reservation scheduled with us, please take a moment to read our Boarding Agreement in its entirety and complete the highlighted areas of the form. Please note that there are areas where it's appropriate to indicate if you wish for your pet to have ancillary services, such as a bath, grooming or vaccinations, completed while staying with us.

New Client Form

If you are new of our office, please take a moment to complete our New Client Form. Be sure to include a current email address, as this is how all your pet's reminders will be delivered to you.

Grooming Release Form (for new grooming clients only)

If your pet has an upcoming spa day scheduled with our fantastic groomer, Kandy, please take a moment to complete this form. This form will give Kandy some helpful information about you pet's past experiences and what your wishes are for his/her groom.

Drop Off Information Sheet

If you are planning to leave your pet with us for the day so that Dr. Tucker can perform an exam and any subsequent procedures, please take a moment to complete our Drop Off Information Sheet. Please provide as much detailed information as possible. Make sure to include one or two good phone numbers where Dr. Tucker or a staff member can contact you throughout the day and circle 'yes' or 'no' to the question at the bottom.

Medical History Form(complete for first time visits, or at your pet's annual or bi-annual wellness exam)

If your pet is a first time patient to our office, or if it's time for his/her annual or bi-annual wellness exam, please take a moment to complete the Medical History Form. This form will alert our staff to any changes in your pet's environment, behavior, physiological state or other important factors. Knowing this information helps our office tailor a wellness plan that meets the needs of each individual pet in our care.
